Our practice is committed to providing counseling services that are ethical, respectful, and centered on your well-being. As a Licensed Professional Counselor (LPC) practice, we adhere to professional standards established by the Behavioral Health Executive Council (BHEC) and strive to create a safe, supportive, and confidential environment for care.
Protecting your privacy, honoring your rights, and responding thoughtfully to questions or concerns are fundamental to our work. We take our ethical and legal responsibilities seriously and are committed to providing services with integrity, accountability, and compassion. Part of that ethical and legal responsibility is ensuring you understand your health rights as our client. Additionally, the information below is located in your first appointment paperwork and here in the office.

Your rights regarding your health information
We are required by law to maintain the privacy of your protected health information (“PHI”) and to provide notice of our legal duties and privacy practices under the Health Insurance Portability and Accountability Act (HIPAA) and the Texas Health & Safety Code, Chapter 181.

As a client or consumer, you have the following rights:
1. Right to Access and Obtain Copies of Your Counseling Records
You have the right to inspect and obtain a copy of your counseling and health care records maintained by this practice, with certain limited exceptions permitted by law.

How to exercise this right:

Submit a written request identifying the records you wish to access or receive

Requests may be made for paper or electronic copies, where available

We may require verification of your identity before releasing records

Requests will be processed within the timeframes required by HIPAA and Texas law

Reasonable, cost-based fees may apply as permitted by law

2. Right to contact the licensing or disciplinary authority
If you believe an LPC has violated professional standards, ethical obligations, or applicable laws, you have the right to contact the licensing or disciplinary authority responsible for regulating Licensed Professional Counselors in Texas.
Licensed Professional Counselors are regulated by the Behavioral Health Executive Council (BHEC).

3. Right to file a privacy or consumer complaint
(HIPAA and Texas Health & Safety Code §181.103)
You have the right to file a complaint if you believe your privacy rights under HIPAA or the Texas Health & Safety Code have been violated.
Complaints to the Behavioral Health Executive Council must be submitted using the Council’s official complaint form.

4. No retaliation
We will not retaliate against you for requesting access to your records, filing a complaint, or exercising any rights provided under HIPAA, Texas law, or LPC professional standards.
